Hampden County Correctional and Community Health Program, Hampden County Sheriffs Department
Hampden County Correctional Center uses a public health model for preventing, detecting, and treating various health issues among people who are incarcerated during the period of incarceration and following their release to the community. Members of four jail health teams, which are integrated with four community health centers, conduct an early intake assessment of each person's medical, mental health, and dental needs and enter this information into a shared electronic medical record. When an individual is within 30-60 days of release, medical discharge planners prepare a prerelease plan that includes, when necessary, medication to cover five to seven days, a prescription, and a follow-up appointment at the individual’s local community health center.
